Recipient Information

All C2Y carrier recipients must agree to the following agreement:

Carrier Loan Agreement

This agreement is between Close 2 You (lender) and ___________________ (recipient).

Congratulations on being selected to receive a carrier through Close 2 You. Prior to receiving your carrier, please carefully review, sign and date the Carrier Loan Agreement. By signing this document you agree to Close 2 You procedures and regulations. Read and initial each statement.

_____ 1. I understand that the C2Y carrier (Tula, KinderPack or other monogrammed carrier) is property of Close 2 You.

_____ 2. I will not alter the structural integrity of the carrier by modifying it. I am allowed to add accessories that do not alter the structure of the carrier: drool pads, and hood.

_____ 3. I agree to keep my Facebook profile active, be a member of the Close 2 You Facebook Group, and a friend of C2Y Verification for the duration of the loan.

_____ 4. I agree to timely submit photos of the C2Y carrier with a dated piece of paper when requested by a C2Y admin.

_____ 5. I will return the carrier to C2Y.

1. The carrier must be returned one year from the signing of this document OR
2. The carrier must be returned if I become ineligible as a C2Y recipient

_____ 6. I will not smoke or wear heavy scents will wearing or being in the vicinity of the carrier. I will also keep any household pets away from the carrier. This is to protect the next recipient should they suffer from allergies and chemical sensitivities

_____ 7. Should I win or be gifted a carrier while in possession of a Close 2 You carrier, I will immediately notify Close 2 You and make arrangements to send the carrier back

_____ 8. Should my financial situation change, allowing me to purchase a carrier, I will return the carrier to Close 2 You, so that another family will be able to use it.

_____ 9. I understand that it will take 3-5 weeks for the carrier to arrive.
